Payment Options:
For patients who do not have medical insurance coverage or are covered by the few HMO policies that we currently do not accept, we have discounted payment options available. Please contact our office for details.
Appointments:

For patients covered by Workers' Compensation, please ask to speak to the Referral Coordinator.
Physician Referrals may be called in or faxed to 770-506-4686. Physician Offices, click here to print a Provider Fax Referral Form, or you may use your own.
When a faxed referral is received, we will contact the patient to schedule an appointment. Once the appointment has been made, we will contact your office with the date and time of the appointment.
Emergencies:
If you are experiencing a life-threatening emergency and/or are in severe pain and require immediate medical attention, please dial 911 or proceed to the nearest emergency medical facility.

By sending your injured employees to us first, when appropriate, you are eliminating a timely and costly step, as it is likely the patient may be referred to us eventually. Should a patient be sent to us first and is in need of additional care, we will work with you to send the patient to the proper provider. We specialize in treating acute and chronic pain using the latest treatment options.

Our goal is to reduce and in most cases eliminate the injured employee's pain through a multidisciplinary approach, allowing the employee to return to work in a timely manner.
